Definition

A High Vaginal Swab (HVS) test is a laboratory investigation in which a sterile cotton swab is used to collect a sample of vaginal discharge from the upper part of the vagina (posterior fornix). The sample is then examined under a microscope and cultured to identify microorganisms causing vaginal infections and to determine the most effective antimicrobial treatment.

Purpose of the HVS Test

The HVS test is performed to:

  • Diagnose vaginal infections.
  • Identify the microorganism causing abnormal vaginal discharge.
  • Determine whether the infection is bacterial, fungal, or parasitic.
  • Guide appropriate antibiotic or antifungal treatment through culture and sensitivity testing.
  • Monitor recurrent or persistent vaginal infections.

Indications (When is the HVS Test Recommended?)

A healthcare provider may recommend an HVS test if a woman has:

  • Abnormal vaginal discharge
  • Foul-smelling vaginal discharge
  • Vaginal itching or irritation
  • Burning sensation in the vagina
  • Pain during sexual intercourse (dyspareunia)
  • Lower abdominal or pelvic pain
  • Suspected bacterial vaginosis, candidiasis, or trichomoniasis
  • Recurrent vaginal infections
  • Persistent symptoms despite treatment

 

Principle of the Test

The test is based on collecting microorganisms present in vaginal secretions. The collected sample is examined by:

  1. Microscopy – Detects bacteria, fungi, parasites, pus cells, epithelial cells, and yeast.
  2. Culture – Allows microorganisms to grow in the laboratory for identification.
  3. Sensitivity Testing (Antibiotic Susceptibility Test) – Determines which antibiotics or antifungal drugs are most effective against the isolated organism.

 

Sample Required

  • High vaginal swab (vaginal secretion)
  • Collected using a sterile cotton swab
  • Placed in a sterile transport medium and sent immediately to the laboratory.

Procedure

  1. The patient lies in the lithotomy position.
  2. A sterile speculum is gently inserted into the vagina.
  3. A sterile swab is used to collect discharge from the upper vagina (posterior fornix).
  4. The swab is placed into a transport tube.
  5. The specimen is transported to the microbiology laboratory.
  6. Laboratory tests including microscopy, culture, and sensitivity are performed.

 

Laboratory Examination

1. Microscopy

Examines:

  • Pus cells (White Blood Cells)
  • Epithelial cells
  • Red Blood Cells (if present)
  • Bacteria
  • Yeast cells
  • Budding yeast
  • Pseudohyphae
  • Trichomonas vaginalis
  • Clue cells (suggestive of bacterial vaginosis)

2. Culture

The sample is cultured on appropriate media to isolate pathogenic microorganisms.

Common organisms isolated include:

  • Candida albicans
  • Gardnerella vaginalis
  • Escherichia coli
  • Staphylococcus aureus
  • Streptococcus agalactiae (Group B Streptococcus)
  • Klebsiella species
  • Proteus species
  • Pseudomonas aeruginosa

3. Antibiotic Sensitivity Test

If bacteria are isolated, susceptibility testing identifies the most effective antibiotics for treatment.

Diseases Diagnosed by HVS Test

  • Bacterial vaginosis (BV)
  • Vulvovaginal candidiasis (Yeast infection)
  • Trichomoniasis
  • Aerobic vaginitis
  • Mixed vaginal infections
  • Other bacterial vaginal infections

Interpretation of Results

Normal Result

  • Normal vaginal flora (predominantly Lactobacillus species)
  • No significant pathogenic organisms
  • Few white blood cells

Abnormal Result

May show:

  • Increased pus cells
  • Presence of pathogenic bacteria
  • Candida yeast cells or pseudohyphae
  • Trichomonas vaginalis
  • Clue cells
  • Heavy bacterial growth on culture

Limitations

  • Does not diagnose all sexually transmitted infections (STIs).

  • Some organisms require additional molecular tests (e.g., PCR).

Precautions Before the Test

  • Avoid antibiotics for 48–72 hours before testing (unless advised otherwise).
  • Avoid vaginal creams or medications for 24–48 hours.
  • Avoid sexual intercourse for 24 hours before the test.
  • Avoid douching before sample collection.
  • If possible, the test should not be performed during menstruation.
